At Southwestern Medical Affiliates, it's our privilege to provide for the healthcare needs of Lawton Fort Sill and the surrounding communities.

Comprised of providers who are employed by Southwestern Medical Center, we provide patients a team approach to their care and a seamless transition between services.

Online Scheduling

Our online scheduling service is fast, safe, convenient and available on any smartphone, tablet, laptop or desktop computer. Our scheduling portal allows you access to see provider availability and reserve an appointment time that works best for you.

When You Visit

To make the most of your appointment, please bring all your prescription medications, over-the-counter medications, vitamins and other supplements with you. It's important your provider know what you take on a regular basis to ensure there are no possible drug interactions and your medications are being taken as directed.

Financial Policy

We request your co-payment or coinsurance portion be paid at the time of service. If you have difficulty meeting
your financial obligations, please call our Billing Office at 580-531-6464 to make payment arrangements. An unpaid, outstanding balances may lead to discharge from our practices.

Because unexpected life events can bring unplanned healthcare expenses, we will make every effort to work with you. For patients with an outstanding balance, we offer financial adjustments based on income. These are addressed on a case-by-case basis.

If you're uninsured, we offer options that can make your treatment more affordable. Some of these options include a discount on balances paid in full at time of service and assistance through the MDSave program.

Prescription Refills

We request all prescription refills be requested by your pharmacy. Our electronic communication with your pharmacy is very secure and allows your refill request to be filled in a more timely manner.
Phone calls to the clinics to request prescription refills may delay the refilling process.

No Show Policy

We understand unplanned events happen in everyone’s lives, from time to time. However, we ask you to please make every effort to call our office if you will be late or unable to make your appointment time.

We allow three (3) “No Show” visits in a 12-month period. Additional “No Show” appointments may lead to being discharged from our practices.
